Waiver, Release, Assumption of Risk, and Indemnity Agreement
Please read carefully. This document affects your legal rights.
I, do hereby submit my WAIVER to SIX BLADES JIU-JITSU NEW MARKET. I hereby assume full responsibility and liability for all damages, injuries and losses that I may sustain or incur while participating in or watching activities within the DOJO.
I realize that serious injury is commonplace in martial arts schools such as the one I am hereby entering; and that even death is a very real possibility. I also realize that if I am injured, I might be disfigured, disabled and/or rendered unable to work again. By entering into this agreement, I realize that regardless of how these consequences might occur, I accept the risk of being present at activities with Six Blades Jiu-Jitsu New Market and give up and waive all claims I or anyone acting on my behalf or through me might have against Six Blades Jiu-Jitsu New Market, and any owners, instructors, employees, members, and guests for any injury regardless of its nature or effect on me as a result of my participation in activities and/or presence at the Dojo.
I authorize, and agree to hold harmless, whatever medical personnel that may be present at the Dojo to take any action necessary, should I become injured. I represent that I am in good health; that I am not presently, nor will participate in activities at the Dojo while under the influence of any drug or medication; that no one affiliated with Six Blades Jiu-Jitsu New Market has made any representations regarding my fitness or ability to participate; that I have read and understood every provision of this release; and that I am legally competent to and freely enter into this waiver, release and assumption of risk agreement.
I further understand that filmmakers, photographers, and spectators may be present at the Dojo and activities there may be videotaped, photographed or otherwise shown to audiences around the world. I hereby consent to having my likeness shown, publicized, commented and/or reported on; and I do not expect to receive, and in fact waive any compensation I might otherwise be entitled to as a result thereof.
COVID ADDENDUM
The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious. By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk not holding any liability towards Six Blades Jiu-Jitsu New Market or its staff. Six Blades Jiu-Jitsu New Market will look to meet and exceed every measure and guideline set for safety and cleanliness but cannot guarantee any members from becoming ill. I hereby release, covenant not to sue, discharge, and hold harmless Six Blades Jiu-Jitsu New Market, its employees, agents, and representatives, of and from claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any claims based on the actions, omissions, or negligence of the Six Blades Jiu-Jitsu New Market, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any Six Blades Jiu-Jitsu New Market program.
I HAVE READ THE FOREGOING WAIVER AND RELEASE OF LIABILITY AND VOLUNTARILY EXECUTED THIS DOCUMENT WITH FULL KNOWLEDGE OF THIS CONTENT.

Please review below and sign (parent and child if applicable) our academy oath, academy rules and academy expectations below as a first step towards continuing your journey with Six Blades Jiu-Jitsu New Market.
ACADEMY OATH
I intend to be gentle, respectful and a good listener. I will always give my best to develop my self-discipline and to have the courage to do what is fair and truthful. I am aware that by learning the art of Jiu-Jitsu my mind and body are being empowered, but I promise not to be abusive nor offensive to others and always pursue friendship.
ACADEMY RULES
- Be respectful to your coaches and training partners.
- Look to set a great example as student and leader.
- Safety is the priority. Use control when training.
- Treat the academy with respect, clean up.
- Bow to the mat before stepping on and off the mat.
- Wear your shoes/sandals while in the bathroom.
- NO foul language or loud outbursts.
- NO eating in training room.
- NO shoes in training room.
- NO coaching your children or other students.
- Wearing rash guards under the Gi is mandatory.
- Be mindful of your hygiene.
- Your uniform must be always clean for classes.
- Keep fingernails and toenails neatly trimmed.
- Remove all jewelry or other items such as hair pins.
- All wounds must be properly covered.
- Do not train if you are sick.
EXPECTATIONS OF STUDENTS AND PARENTS
If you or your children are sick PLEASE be considerate of the coaches and students and stay home!
Parents, if you are sitting in the training room we ask you to NOT coach or talk to your children. We know it’s hard but it is very distracting for your student and other students to have parents talking to students or offering “coaching” from the sides.
Students need to bring their water bottles into the training room and leave them on the floor off the mat or on a bench. Students should NOT be running out to the lobby to get a drink during water breaks.
Parents, please encourage your children to learn to tie their own belt. They need to tie it themselves or wait for a coach to be available. They should NOT be running to a parent off the mat.
Parents please do not leave children unattended before and after classes. If children are not actively participating in a class they need to be with a parent or guardian.
The downstairs is NOT part of our studio space so please treat it with respect and do not leave students unattended to play before and after class.
It’s important students are coming in ready for class and on time. We know life happens but please make an effort to have children arrive on time, dressed and ready at the start time for the class.
We operate with a 3 strike rule. The first strike will be push ups, the second strike we will ask the student to sit out for 5 minutes, the third strike will result in being asked to sit out for the rest of class. If a student is consistently asked to sit out we will evaluate their place in the program.
Behavior issues include but are not limited to:
- Being disrespectful to coaches, other students or parents.
- Rough housing with other students.
- Speaking out of turn.
- Being off task and disrupting class.
- Reckless behavior that results in injuring others.
